What You Will Do
- Oversee an interdisciplinary team of community-based clinicians that deliver quality patient care from admission to discharge.
- Manage, empower, and motivate team members in a dynamic work environment.
- Proactively execute administrative functions to achieve seamless patient care including the review of clinician caseloads, staffing needs, recruitment process, and assigned scheduling of team members.
- Use and learn tools and systems, problem solve new and different situations, and escalate issues timely to leadership to achieve organizational success and improved clinical outcomes.
Licenses and Certifications:
- License and current registration to practice as a Registered Professional Nurse or Physical Therapist in New York State, required
Education:
- Bachelor’s degree in nursing or physical therapy from a program approved by the New York State Department of Education or equivalent work experience or
- Master's degree in nursing or physical therapy or equivalent work experience, required
Work Experience:
- Minimum of three years nursing experience and one year home health experience or
- Minimum of three years of physical therapy experience, required
- Clinical management experience, preferred
