Physical Therapist - Home Health - (PT - HH)

1. How long is the contract assignment? 13 weeks 2. Schedule of assignment hours? M – F, 8am – 5pm, Weekend rotation for PT admissions 3. Will this candidate be visiting patients in care facilities or only in patient’s homes? Both 4. Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? No. 6. What region are you in? Home health or Hospice? Home Health 7. What’s creating the request? - PT leaving to go home for 5.5 weeks 8. To which location will this person report for assignment, full address, Including the OU (Location Code) 9. 303 Quartmaster Ct. How many miles per week will the consultant travel? 30/40 a day? Weekly? 30-100 daily 11. What are the weekly units of productivity expectations? 30 12. What EMR systems will the consultant be using: HomeCare HomeBase, Oasis, Outlook, Teams 13. Will there be any on call? If so, how much? 1 weekend a month 14.
