The University of Miami Health System, "UHealth", Department of Pediatrics-Infectious Disease has an exciting opportunity for a Full-Time Social Worker, LCSW, to work in Miami Florida.
The Social Worker, LCSW works with clients in order to help them deal with issues involving mental and emotional health. This position provides emotional support, mental health evaluations and therapy, and case management services.
Core Responsibilities:
Advocates for and helps clients gain access to resources while confronting their personal issues.
Counsels and provides crisis intervention for patients, including assessment and treatment of behavioral problems.
Coaches patients on how to manage ongoing health conditions.
Provides therapy to help patients make positive behavior changes.
Assists patients and their families through individual, group, and education conferences to understand, accept, and follow treatment recommendations.
Refers patients and their families to specialized treatment programs in accordance with their needs, and monitors and evaluates their progress.
Collaborates with the other departments to integrate care where needed.
Maintains current and appropriate documentation in patient chart.
Acts as a mandated reporter in suspected child abuse cases.
Adheres to University and unit-level policies and procedures and safeguards University assets.
Department Specific Functions
Conduct mental health assessments and psychosocial assessments, including the use of standardized screening and assessment instruments as indicated by the clinical supervisor.
Participates as core member of Physician, Nurse, Social Worker Multidisciplinary Care Team for designated patient population
Attends weekly pre-clinic conferences, updating mental health and psychosocial problem list and communicating interventions as allowed by HIPPA.
Identifies psycho-social care needs, including them in the multidisciplinary individualized, goal-oriented care plan documented in the EMR - UChart.
Develops a written, individualized psychosocial service plan for each client, documented in a secure location, which includes a service needs assessment, community referrals and follow-up to services.
Reassess client needs and modify treatment plans at minimum once every six months.
Provide individual, couple, and family psychotherapies, interventions, and/or services for clients as indicated by the assessments and as specified in the treatment plan.
Conduct support, psychoeducational and/or therapy groups as requested by the program and clinical supervisor.
Provide crisis intervention services to patients as required, including statutory procedures (e.g., psychiatric emergencies, abuse/neglect, HIV reporting, etc.) in consultation with the clinical supervisor.
Conduct home visits on active and potential patients to evaluate home settings and environmental safety, to conduct psychosocial and mental health assessments and/or treatment, and to reinforce adherence to the medical and mental health plans of care.
Initiate and maintain current progress notes on all patients for every encounter, maintain all necessary reports, activity logs and data collection measures as required by the program and clinical supervisors.
Participate in weekly divisional pre-clinic or post-clinic meetings to coordinate patients’ mental health care with medical treatment.
Engage children, youth, women and their families in comprehensive health education including age appropriate sexual and reproductive health information
Participate in all individual and group supervision meetings as required by the clinical supervisor.
Participate in the training and support of graduate students, residents, and other trainees as required.

CORE QUALIFICATIONS:
Master’s degree in social work
Active License Clinical Social Worker.
Minimum 1 year of experience
Ability to handle difficult and stressful situations with professional composure.
Ability to maintain effective interpersonal relationships.

Excellent oral and written English communication skills.
Function as member of multidisciplinary health care team.
