Job Title: Transplant Coordinator Registered Nurse (Transplant Coordinator RN)** Job Summary:
A Transplant Coordinator Registered Nurse (Transplant Coordinator RN) provides specialized care and oversight to patients undergoing organ or tissue transplantation. The Transplant Coordinator RN serves as a key point of contact for patients, ensuring comprehensive care, education, and coordination of all aspects of the transplant process, including evaluation, pre- and post-transplant care, and follow-up.
Key Responsibilities:- Assess and evaluate potential transplant candidates, ensuring they meet medical and clinical criteria for organ or tissue transplantation.
- Collaborate with multidisciplinary teams, including transplant surgeons, physicians, and social workers, to develop individualized care plans for transplant patients.
- Educate patients and their families about the transplant process, including medical procedures, pre- and post-operative care, and long-term management.
- Monitor patients’ progress before and after transplantation, including assessing for potential complications, infections, or rejection.
- Coordinate and facilitate the organ/tissue donation and transplant process, working closely with organ procurement organizations (OPOs) and transplant centers.
- Maintain detailed documentation of patient care, transplant evaluations, and outcomes to ensure compliance with regulatory and clinical standards.
- Provide emotional and psychological support to transplant patients and their families, addressing concerns related to pre-transplant evaluations, surgery, and post-transplant recovery.
- Collaborate with insurance providers to ensure patients have access to transplant-related services, including prior authorizations and reimbursement processes.
- Monitor and coordinate transplant medications, lab work, and follow-up appointments, ensuring patients are adherent to treatment protocols.
- Assist in educating and supporting transplant patients in managing lifelong medications, including immunosuppressants to prevent organ rejection.
- Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
- Licensure:
- Valid Registered Nurse (RN) license in the state of practice.
- Certification:
- Basic Life Support (BLS) certification required.
- Transplant Coordinator Certification (CCTC) preferred.
