St. Luke’s home health team of skilled professionals provides comprehensive health care in the warmth and comfort of the patient’s home. From newborn to seniors, we offer the latest home health techniques and technology to people at all ages and stages of life. Dedicated to the continuum of care as patients transition from inpatient to home to outpatient settings for care.
Therapy Liaisons are a critical role for the success of all post-acute care. A successful liaison will have great people skills, be able to build trusting relationships with hospital staff and providers and have clinical understanding for many acute complex cases that require continued care in the home. You will coordinate care within the Twin Falls area.
What You Can Expect:
You will be responsible in coordinating home care for patients that have been newly diagnosed, discharged from the ER, prescribed new medications, require respiratory treatment and wound care. Much of the Liaison's time is spent by performing assessments and screening, establishing patients’ eligibility for care, reviewing referrals for appropriate placement, meeting with patients/families to describe our services and educate them on the benefits of home care. You will also work closely with providers and staff for other possible referrals.
- Full Time Days (Monday-Friday 8-hour shifts) with rotating weekend call
- Mileage reimbursement
- Coordinates and facilitates referrals to St Luke’s based on admission criteria.
- Assist in the information gathering, education and form completion aspects of patient intake, admissions, and transitions to inpatient facilities or home.
- Represent the organization and participate in marketing and education programs to increase awareness across the system
- Qualifications:
- Education: Degree from an Accredited Program
- Experience: 3 years relevant experience
- Licenses/Certifications: Valid license to practice audiology, physical, occupational or speech therapy in state of practice
- Valid Driver's License and proof of Auto Insurance
- Preferred Qualifications:
- Occupational or Physical Therapist
- Home Health Experience
