Occupational Therapist - Home Health - (OT - HH)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $2039.58 - $2349.96

Organizes and conducts prescribed skilled occupational therapy programs through the application of purposeful, goal-directed activity in the treatment of persons whose function is impaired by physical illness or injury, emotional disorder, congenital or developmental disability or the aging process in order to achieve the optimum functioning, to prevent disability and to maintain health. Required Minimum Knowledge, Skills, and Abilities (KSAs) 1. Education: Entry level degree in Occupational Therapy. 2. License/Certifications: State of Maine licensure as an Occupational Therapist. Current, valid driver’s license to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy. Current certification in Basic Life Support for Healthcare providers. 3. Experience: Minimum one year of full-time equivalent experience in a supervised setting. 4. Strong interpersonal skills. 5. Sense of humor. 6. Excellent composition skills. 7. Creativity. 8. Ethical Reasoning. Essential Functions Accurately develops and plans programs involving activities of daily living; the design, fabrication and application of orthoses (splints); sensorimotor activities; therapeutic use of activities and the activity process to develop/restore functional performance; guidance in selection and use of adaptive equipment; pre-vocational evaluation and training; consultation concerning the adaptation of the physical environment to the handicapped/disabled; and community re-entry. Reviews and assesses each referral for OT services. Accurately selects appropriate evaluation tools. Thoroughly evaluates each patient's current level of function. Evaluation tools include specific tests, interviews of patient and/or family, and observations. Critically analyzes evaluation results and determines a plan of care, which includes treatment options, treatment priorities, short-term and long-term goals, precautions, schedule and frequency for each patient. -Proficiently implements the plan of care including such modalities/activities to meet treatment objectives as: physical daily living skills, work skills, sensory integration activities; energy conservation/joint protection; gross and fine motor coordination; neuromuscular/strengthening activities; fabrication of orthotics/assistive devices. Effectively teaches and instructs the patient, dependent upon patient's level of understanding, about his/her mutual responsibility in implementing the plan of care. Consistently collaborates with patient to determine treatment effectiveness, and to determine any alteration inpatient's attitude/behavior and comprehension of treatment program. Periodically re-assesses clinical outcomes of the patient and revises/modifies the plan of care as indicated. Discharges/terminates patient treatment when maximum function has been obtained or when no further progress can be attained. Provides accurate, complete and timely documentation related to each patient according to hospital guidelines and to third party payer requirements. Daily records patient's attendance. Accurately records results of OT evaluation. Formulates initial evaluation/assessment which documents the patient function/dysfunction, short-term and long-term goals in functional terms with time frames and treatment options. Maintains a complete progress record of the patient's response to treatment. Completes all appropriate certification forms monthly, i.e. Medicare, CHAMPUS, Managed-Care. Summarizes patient course of treatment periodically to referring physician. JCAHO recommends every two weeks for inpatients and monthly for outpatients. Maintains a copy or record of home program in the patient's record. Concisely summarizes a patient's course of treatment in each discharge summary. Recommends/designs/fabricates/modifies static/dynamic splints, home/work environments and other small orthotic devices to alleviate/substitute for lack of body function. Assesses patient's current skill level and the parameters of specific durable medical equipment. Suggests/adapts special equipment accordingly. Critically re-assesses functional outcome of patient's performance. Properly supervises occupational therapy assistants, aides or clinical students in patient-related activities. Adheres to the delegation of responsibility guidelines set by the Maine Department of Business Regulation and the American Occupational Therapy Association. Familiarizes self with the job description of assistants and aides. Participates in the departmental orientation of new employees when requested. Contributes to annual performance appraisal of COTA's and aides. Trains and evaluates clinical students according to the approved clinical training program of the department. Maintains ongoing communication and collaboration with treatment team members to select activities and provide programs consistent with patient needs. Shares pertinent information with nursing, physician or other health care provider through patient conferences, verbal contacts and/or patient records. Establishes dialogue with outside resources to maximize patient's success, e.g., CHANS, rehab facilitates, etc. Presents an accurate representative picture of a patient's capabilities at family meetings, IEP's quarterly reviews. Coordinates patient's schedule in conjunction with other health services if possible. Maintains a clean, organized and hazard free work environment. Remains knowledgeable of all safety, infection control and fire/disaster policies and procedures. Safely and properly operates all treatment equipment. Keeps patient areas free of obstacles. Returns all patient equipment to designated areas for the convenience of other staff members. Remains alert to any adverse reactions that patients may experience due to temperature, odor or location/position. Notifies aide of shortage in departmental supplies/inventory. Notifies supervisor and Physical Plant if there is any mechanical failure with departmental equipment. Participates in monitoring, evaluating and improving the quality and appropriateness of patient care at MCHS. Suggests constructive improvements in the services to achieve departmental goals. Participates in the overall management of the department. Regularly attends and participates in staff meetings. Contributes yearly to the budget planning. Participates in interviewing new departmental employees. Professionally represents the department throughout the MCHS system. Promotes a positive image through neat attire and supportive attitude toward staff, patients and families. Promotes a team approach to patient care. Upholds confidentiality of patient information at all times both inside and outside the hospital setting. Assumes responsibility for continuing competency in adherence to current standards of care. Maintains professional license to practice. Attends/participates in two continuing education offerings a year. Shares new information during departmental in-services. Sets annual professional goals and objectives. Keeps supervisor informed of progress toward goals. Competent in age specific development as evidenced by competency program for the population served as documented in the department specific scope of service. Problem Solving and Analysis Employee is responsible for the evaluation, interpretation, planning and implementation of patient care issues. Therapist must be able to differentiate symptoms/behaviors and adjust plan accordingly. Analysis and corroboration with the physician is required. Establishing priorities and implementing a logical plan is essential. Must be able to formulate suggestions regarding community resources.
