Job Description

The position requires a Registered Nurse (RN) to serve as a Case Manager, focusing on care coordination and transitions planning throughout the acute care patient experience. The role involves collaboration with various healthcare professionals to ensure timely patient progression and transitions to appropriate levels of care.

Key Responsibilities

• Conduct initial and continued patient assessments.
• Collaborate with interdisciplinary teams to develop comprehensive care coordination plans.
• Monitor patient length of stay and ensure appropriate documentation supports level of care determinations.
• Formulate transition plans and proactively identify barriers to care progression.
• Participate in patient rounds and ensure timely transitions to lower levels of care.
• Communicate effectively with patients, families, and healthcare teams.

Experience Requirements

• Minimum of 5 years of RN Case Management experience.
• Current California RN License.
• Basic Life Support (BLS) certification.

Certification Requirements

• Advanced Cardiac Life Support (ACLS)
• Pediatric Advanced Life Support (PALS)
• Neonatal Resuscitation Procedure (NRP)
• Critical Care RN (CCRN)
• Trauma Nursing Core Course (TNCC)
• Electronic Fetal Monitoring (EFM)
• Additional certifications as applicable to the role.
