Required Credentials and Certifications

  1. BLS Certification

    • Accepted from AHA, Red Cross, CHSF, or Military.
    • Preferred: AHA certification.
  2. License/Certificate Verification

    • Primary Source Verification of professional license and/or certification required within 30 days of start date.
    • MA Conditional Licenses not accepted.
  3. MGB Medication Exam

    • 80% Passing within 365 days for RN & LPN.
  4. COVID-19 Vaccine

    • Completed primary vaccination series approved by the FDA or WHO required.
    • No exemptions or waivers considered.
  5. MMR Vaccination

    • Documentation of two measles, two mumps, and one rubella vaccine required.
    • Proof of immunity via blood test accepted.
  6. Influenza Vaccine

    • Required during flu season (Sept 15th - April 30th).
    • No exemptions or waivers considered.
  7. TB Test

    • Documentation of TB skin test screening or negative QuantiFERON TB Test required within 3 months of start date.
  8. Health Statement

    • Physician statement signed by MD/DO/NP/PA required within 365 days of start date.
  9. MGB Criminal Background Check

    • Must be completed within 45 days prior to start date.
  10. MGB Massachusetts CORI

    • Attestation indicating date completed and findings required within 30 days prior to start date.
  11. MGB Resume

    • Include past 7 years of work experience with city & state.
  12. MGB References

    • 2 recent references required, with at least 1 from a supervisor or above.
  13. MGB Skills Checklist

    • Required for submission.
  14. MGB Confirmation of Assignment/POI Form

    • Required upon acceptance of assignment.
  15. Education Verification

    • Verification of highest level of education achieved required.
  16. MGB National Sex Offender Check

    • Conducted within 30 days of start and annually.
  17. MGB Exclusion Checks

    • Search OIG, GSA, and MassHealth & NH Medicaid exclusion lists required.

Additional Information

  • Preferred: Documentation of Hepatitis B vaccination or proof of immunity.
  • Preferred: Tdap vaccine within the last 10 years.
  • TB Screening Form: Must be completed prior to start and updated annually if applicable.
