This large Level 1 Trauma center located in heart of Mississippi is looking for Social Work Case Managers to work 8hr days. Must have level 1/large medical center experience in Inpatient/Acute Care Case Management/Discharge Planning.

Must be SW Case manager with at least 2 years Acute Care Management experience in Discharge Planning with previous experience in large academic Medical Center hospital. Must have general understanding of discharge readiness.

Shifts: M-F 8-430p. No Weekend Requirements. Holiday Requirements are Rotated as needed.

Fingerprint requirement can be done remotely (takes approx 10-14 DAYS) or in person in Jackson, MS 1 week prior to assignment. FLU VACCINE IS REQUIRED at UMMC - only medical exemption is allowed and requires documented evidence from a health care provider of a past severe reaction to the vaccine.
