Job details

Sub requirements - Coversheet, resume, primary source verification, skills checklist, BLS and ACLS.
Staff required to wear lead.
Includes weekends and on call. No holidays except for on call.
Staff will be expected to float assignments within the Radiology and Pain departments.
COOK IS A MANDITORY FLOAT FACILITY, all resources are required to float at the facilities discretion.
RTO: 5 days max with no more than 2 consecutive. No RTO the day prior or after a holiday. No RTO on weekends.

Government-Issued photo ID. Color or B&W acceptable. Must be clear and legible. Back of card only required if it contains pertinent information.
Refer to UD/cost center to confirm certifications required per unit.
