Job Description

The position requires a Registered Nurse (RN) specializing in Case Management. The RN will be responsible for care coordination and transitions planning throughout the acute care patient experience. This role involves collaboration with physicians, utilization managers, medical social workers, and bedside RNs to ensure timely patient progression and transitions to appropriate care levels.

Key Responsibilities

• Conduct initial and continued patient assessments.
• Develop comprehensive care coordination plans.
• Monitor patient length of stay and ensure appropriate level of care.
• Collaborate with interdisciplinary teams to facilitate timely transitions.
• Address complex clinical and social situations to prevent unnecessary admissions.
• Communicate effectively with patients, families, and healthcare teams.

Experience Requirements

• Minimum of 5 years of RN Case Management experience.
• Current California RN License.
• Basic Life Support (BLS) certification.

Work Schedule

• 5/8 shifts
• Every other weekend

Certifications Required

• CA RN License
• BLS
• Additional certifications may include ACLS, PALS, NRP, CCRN, TNCC, and others as applicable.

Additional Information

• Contract length: 13 weeks
• Candidates must complete various health documents and background checks prior to starting.
