Additional Details

Government issued photo ID required. Photo Identification must be carried on person during hours of service to client.
Please require if nurse is working on a compact license.
Orientation and Annual Education - Must review prior to start and complete the Attestation form under Facility Specifics.
Confidentiality Agreement - Must complete prior to start
Candidate must submit if they are seeking a religious or medical exemption. Exemptions must be approved by facility prior to start.
N95 fit test will be completed onsite at orientation. Male candidates MUST be clean shaven for the respiratory fit test.
Candidate will need to create an account prior to start in order to clock in at Cabell.
Any candidates who have not provided proof of COVID vaccinations or do not have the updated 2023-2024 COVID vaccines are required to complete the Cabell Huntington COVID declination form.
