- Will this traveler need to float between like units: yes, floating as needed between discharge locations
- LCSW with discharge experience required
- Independent problem solver, clinical social worker preferred
- Complex discharge experience mandatory
- Must work well with the formal discharge planner on site
- Providing post care planning with the patient
- This position is part of the complex discharge pilot/enhanced care management
- Experience in guardianship, working with DHS
- Experience with brokering with outpatient, housing, homelessness preferred
- 1 year of experience required, new grads with clinical social work experience accepted
- First-timers accepted with the right experience
- Start date: ASAP
- Shift: 5x8HR shifts (M-F 08:00-16:30)
- Weekend work possibly required
- Unit details: New discharge planning program in Washington
- Prefer experience in guardianship, engaging with DHS and managing outpatient transitions
- Daily workload expected to be an average of 15 patients
- Local travel required, may need to go between Colby Campus/Pacific Campus/other local shelters for patient care and follow up
- Not accepting locals at this time, 50 mile radius rule applies
- Pending License not accepted, WA license must be in hand at apply
- Must work 2 of the 3 winter holidays
