Primary Source Verification of professional license and/or certification as required for the position. Ran within 30 days of start. Updated upon expiration.
Proof of 2 Vaccines or Positive Titer showing immunity
Annual Respirator fit test. Locations that allow onsite testing: a number of medical centers.
Flu Vaccine or declination required from October 1 - May 31. Declinations not accepted at certain medical centers.
TB Test - Declinations not accepted at certain medical centers.
Physical/Health Statement - must be done annually, except at certain medical centers where it must be done within 60 days of start and must state clear to work.
National Criminal Background Check within one year of start date. Additional specific requirements for certain states.
Agency attestation accepted
Must score 80% or higher except in Georgia at SRGA must score 85% or higher.
