Required Certifications

• BLS: American Heart Association certification required for all direct patient care personnel.
• ACLS: Required for RNs in CCU, ED, PACU, MICU, SICU.
• PALS: Certificate required for RNs in ED; Emergency Nurse Pediatric Course (ENPC) certificate is also acceptable.

Additional Requirements

• License/Certificate Verification: Must be dated within 30 days of start.
• Fit Test: Required.
• Trinity 9 Panel Drug Screen: Must be completed within 30 days of start.
• COVID-19 Vaccine: Proof of vaccination required.
• MMR: Proof of vaccination required.
• Hepatitis B: Proof of vaccination required.
• Influenza Vaccine: Proof of vaccination required during the flu season.
• Varicella: Proof of vaccination required.
• Tdap: Current vaccination required.
• Physical/Health Statement: Documentation from an exam within the last year required.
• Trinity Profile: Resume, Skills Checklist within a year, and 2 Professional References required.
• Background Checks: Federal, Nationwide, County Criminal from the last 7 years required.
• Education Verification: Must provide documentation of highest level of academic education.
• Employment Verification: Required from the last 7 years.

Compliance and Training

• Trinity Code of Conduct Form: Must be signed and on file.
• Trinity Confidentiality Agreement: Must be signed and on file.
• Trinity System Access Form: Must be completed and uploaded one week before start.
• Trinity Child and Adult Dependent Abuse Training: Required for IA only.
• Trinity Joint Commission Testing: Must be completed within a year.
