Trinity 9 Panel Drug Screen completed within 30 days of start date.

Trinity Code of Conduct Form must be signed.

Trinity I9 must be completed by the agency.

Trinity Confidentiality of Patient and Proprietary Information Agreement must be signed.

Trinity Specialty Specific Competency Testing must be completed within a year.

Trinity Color Vision assessment must be completed.

2 Step PPD OR IGRA (Quantiferon/Tspot) test required.

COVID-19 Vaccine proof required.

MMR proof of vaccination required.

Hepatitis B proof of vaccination required.

Influenza Vaccine proof required.

Varicella proof of vaccination required.

Tdap proof of vaccination required.

Physical/Health Statement must be provided.

Resume and Skills Checklist within a year.

Trinity Conflict Of Interest Disclosure Form must be signed.

Trinity Criminal Background Check must be completed.

Trinity Education Verification required.

Trinity EPLS report required.

Trinity GSA/OIG report required.

Trinity NSO check required.

Trinity System Access Form must be completed one week before start.
