Job Description

The RN will coordinate the management of care, ensuring optimal resource utilization, service delivery, and compliance with external review agencies. Responsibilities include:

• Comprehensive assessment and care planning
• Implementation and evaluation of individual patient needs
• Enhancing patient management quality and satisfaction
• Promoting continuity of care and cost-effectiveness
• Serving as a patient advocate through resource utilization and discharge planning

Facility Requirements

• Certifications:

  • BLS certification (must be through the American Heart Association)
  • RN Case Management experience in a long-term acute care setting
  • Washington RN license
  • 6 hours of Suicide Prevention Training completed through the WA DOH website

• Work Schedule:

  • Monday to Friday, 8:00 AM - 4:30 PM
