Job Description

The Home Health Nurse is a licensed professional responsible for administering skilled nursing care in patients' homes. This role involves collaboration with a multidisciplinary team and adherence to physician orders under the supervision of a Clinical Supervisor.

Responsibilities

• Provide skilled nursing care and comprehensive assessments
• Teach, supervise, and counsel clients and caregivers
• Collaborate with interdisciplinary teams
• Utilize problem-solving and critical thinking skills
• Monitor patient progress to achieve goals
• Complete Oasis documentation
• Administer a range of physician-ordered treatments, including:

  • PleurX Drain
  • Finger stick coagulation checks
  • Wound care and wound vacs
  • IVs and phlebotomy
  • Trachs, Peg Tubes, and Ostomies
  • Telehealth services

Work Schedule

• Days: Monday to Friday
• Hours: 7:30 AM - 4:00 PM

Additional Information

• Experience with Homecare Homebase is preferred.
