- Covid vaccine required, provide vaccination card with submission.
- Documentation of 2 MMRs at least 28 days apart and/or positive laboratory titer
- Fit Test completed onsite, completed annually.
- Hepatitis B (Hep B) - Documentation of Hep B vaccinations (series of 3) and/or positive laboratory titer or signed declination if choosing not to take vaccine.
- Influenza vaccine status required. Declination will be reviewed for religious or medical exemption only.
- 2 Negative TB skin tests. 1 within 90 days and 1 within 365 days of start (TB Skin tests must state negative results, read in MM, dates placed and read and name/title of HCP administering test) or Quantiferon Gold within 90 days, updated annually
- Documentation of 2 doses of varicella 4 weeks apart and/or positive laboratory titer
- Proof of having received Tdap vaccine. The TD vaccine does not meet this requirement.
- Physical Capacity. Written statement by a person lawfully authorized to perform the examination that Resource is physically capable of performing the essential functions of the job. This health exam must be performed within one (1) year prior to assignment to a Customer.
- Passing score of 80% or higher on Norton Specialty Exam, updated every 3 years
- Standard 10 panel drug screen, completed within 12 months pre-start and updated annually
- Background check including County Reside and Employed Search, National Criminal, FACIS level 3, OFAC, OIG Search, SAM, Social Security Trace, NSOP. Education verification required as part of background check, completed prior to start and updated every 3 years thereafter
- Norton Employment Eligibility verification and supporting documentation completed prior to start
- I-9 with supporting documentation completed prior to start
- Physical/Health Statement required.
