Proof of vaccine and 1 booster or exemptions must be submitted for approval

Proof of 2 immunizations, positive IGG titer, if negative titer, must have booster or Declination accepted for MMR

Proof of 3 immunizations and positive antibody titer, if negative titer, booster or Declination accepted for Hepatitis B

Required from October - March (unless otherwise indicated by CDPH) Medical/Religious exemptions will be sent for approval with a physician's statement(medical) or written statement from traveler(religious) for Influenza Vaccine

Proof of vaccination within past 10 years or Declination accepted for Tdap

Required on start date for clinical staff for Covid Test

HLC Testing to be completed by Friday before Start. If completed and confirmation provided Friday prior to start, a $300 stipend will be paid
