Job requirements

Primary Source Verification of professional license and/or certification required.
Proof of COVID Vaccine series OR Bivalent booster required.
Proof of 2 immunizations or positive titer for MMR required.
Fit Test for N95 mask is required.
Hepatitis B Vaccination Series or Positive Titer required.
Influenza Vaccine documentation required from September-May.
If history of positive PPD/TB blood test, a Negative Chest X-Ray and Annual TB Questionnaire are required.
Copy Valid Drivers License required.
2 Step PPD OR IGRA (Quantiferon/Tspot) required.
Proof of 2 immunizations or Positive Titer for Varicella required.
Proof of Tdap Immunization within 10 years required.

Additional Details

Proof of COVID Vaccine series OR Bivalent booster required; Medical exemption reviewed and approved prior to start.
Hepatitis B Vaccination Series, Proof of Immunization, Fit Test, and other medical documents must include identifiable information.
Federal, County, National and Statewide Background Check required.
