Job requires the ability to perform clinical/technical/service/administrative tasks

Responsibilities include assessing patient physical, psychosocial and spiritual health status to determine healthcare and educational needs

Providing and/or delegating patient care according to care goals and setting priorities based on patient needs

Assists with triage of sick and walk-in patients

Establishes therapeutic and supportive relationships with patients and family

Acts as a resource person and coordinates client care, including referral back to primary provider

Involves in telephone triage, handling prescription requests, and communicating test results

Performs therapeutic treatments and assists with procedures

Administers medications and coordinates all patient care activities

Coordination of in-patient admissions and ER referrals

Advocacy for patient needs and ensures patient safety

Participation in performance improvement activities and educational presentations

Works as a member of the patient care team and engages in daily huddles and practice team meetings

Attends in-service and continuing education programs

Displays professionalism and courteous demeanor with patients and team

Informs and educates patients/families for participatory care

Evaluates intervention effectiveness and patient responses

Collaborates with team for patient care adjustments

Communicates patient status changes to care team and health care providers

COVID Vaccine required

BSN required

Minimum $1.25 an hour for parking

Ensures all staff participates in patient care

Effectively communicates with patients, families, and team for care coordination
