Job Description

The Physical Therapist (PT) will:

• Evaluate the physical therapy needs of assigned patients.
• Establish goals with patients and develop individual physical therapy programs to improve mobility and independent function.
• Provide age and developmentally appropriate care to patients ranging from pediatric to geriatric.
• Assess, plan, treat, evaluate, and implement a plan of care (POC) for all clients.
• Demonstrate understanding of the admission process, treatment costs, coding, financial support, and appropriate billing.
• Communicate effectively with patients, families, physicians, and staff, reflecting the hospital's core values.
• Complete documentation of all patient care activities in a timely manner in the electronic medical record.
• Assist in transferring and positioning patients.

Requirements

• Experience: 1 year of Home Health PT experience (required).
• Licenses/Certifications:

  • Valid driver's license and car insurance (required).
  • COVID vaccination (required; declinations will not be approved).
  • OASIS and start of care experience (strongly preferred).
