Job Description

The Therapy OT Home Health position involves coordinating and directing home nursing care according to physician orders and agency policies. Responsibilities include supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.

Key Responsibilities:

• Conduct home visits and provide therapy services.
• Average patient load: 5-8 patients per day.
• Types of cases include Start of Care, Wound Care, and Follow-up Visits.
• Must have reliable transportation and current driver's license.

Requirements:

• Experience: Minimum of 1 year of Adult Home Health or Hospice experience.
• Certifications: CPR and BLS required.
• On-call Requirements: Varies by location and profession.
• Travel: Must be willing to travel up to 75 miles from the home clinic.
• Local Candidates: Accepted at the same rate.

Work Schedule:

• Standard hours: Monday to Friday, 8 AM to 5 PM.
• Weekend requirements vary by location and profession.

Productivity Expectations:

• Weekly units of productivity: 25-30 points.
• Points are assigned based on visit types and activities.

Additional Information

• Dress Code: Scrubs.
• Charting System: Proficiency in HomeCare HomeBase (HCHB) required.
• Background Checks: Comprehensive background checks are required, including state-specific searches.

Health Requirements:

• Physical exam within 12 months of start.
• Negative TB test or chest X-ray required.
• Vaccination documentation for MMR, Hepatitis B, and COVID-19.
