Certification details
- This M-F 40 hour position. Case managers work either 7-3:30 or 8-4:30. BJH Case Managers cover all units at BJH except psych and women and infants. Experience with EPIC and Allscripts preferred. Tasks of BJH Case managers include: completing assessments on all admitted patients, attending interdisciplinary rounds, initiating referrals for home health, dme, skilled nursing facilities, LTACHS and IRFS.
- Avg. Daily Census: 1000+ hospital beds
- M-F; Will work every other weekend
- Unit does not accept 1st time Traveler
- 2 years experience required in case management in a facility of like capacity (number of beds 1000+)
- Academic experience preferred
