Position Summary

This role involves working closely with an inpatient multidisciplinary care team to provide integrated social services care management for patients of all ages. Key responsibilities include:

• Collaborating with patients and families for discharge planning and identifying barriers to discharge.
• Assisting with financial and housing needs, and working with state and federal agencies for placement.
• Providing supportive counseling and linking patients to community services.
• Conducting swing bed admission assessments and ensuring patient understanding of care requirements.
• Facilitating patient/family care conferences to enhance understanding of health conditions and care plans.
• Assisting with legal and financial matters, long-term care needs, and community resource identification.
• Maintaining accurate records and participating in multidisciplinary team meetings.

Education and Experience

• Required: BSW or MSW in social services.
• Experience: Minimum of 2 years in a hospital environment in social work.
• License: Valid State of Alaska Driver's License.
• Age Requirement: Must be 21 years or older.

Knowledge, Skills & Abilities

• Understanding of nutrition, physical activity, and tobacco cessation for health and disease prevention.
• Knowledge of case management principles in integrated settings.
• Familiarity with community support resources.

Skills

• Strong communication skills for effective interaction with patients and families.
• Proficiency in using electronic health records and other relevant software.
• Empathic listening and interpersonal relationship development.
