- Neuro, Cardiac, or vascular surgery needed. IR experience: diagnostic and interventional procedures including and not limited to angiography, embolization, venous access, venography, myelography, abdominal interventions, paracentesis, thoracentesis, biopsy, TIPS, dialysis access/fistula work, Y90/SIRT experience.
- N95 - within 365 days to start date and updated annually. No fit testing(initial or updates) can be done onsite at any location. Fit Test must be completed on the MGB N95 Respirator Fit Test Record Form available in Doc Library. Refer to MGB Fit Test Policy in Doc Library regarding possible exemptions.
- A completed primary COVID-19 vaccination series that is approved by the FDA or the World Health Organization (WHO) OR an up-to-date COVID vaccine if a primary series was never received or completed is required.
- Documentation of two measles vaccines, two mumps vaccine, and one rubella (German measles) vaccines or two MMR vaccines.
- Documentation of TB skin test screening within 3 months of start date and updated if >6 month break.
- Physician Statement signed by MD/DO/NP/PA. Within 365 days of start date and updated if >6 month break.
- Vaccine required during flu season Sept 15th - April 30th.
- If positive on a TB skin test or TB Titer, documentation of a chest x-ray (dated after the positive TB Test) which indicates there is no active tuberculosis is required.
- TB Screening Form to determine there are no symptoms suggestive of TB infection must be completed prior to start and updated annually.
