Required Credentials

1. BLS Certification: AHA, Red Cross, CHSF, or Military accepted.
2. License/Certificate Verification: Primary source verification of professional license and/or certification required.
3. Fit Test: N95 fit test required within 365 days of start date.
4. MGB Medication Exam: 80% passing score required within 365 days.
5. COVID-19 Vaccine: Completed primary series or up-to-date vaccine required.
6. MMR Vaccination: Documentation of two measles, two mumps, and one rubella vaccine or proof of immunity.
7. Influenza Vaccine: Required during flu season (Sept 15 - April 30).
8. TB Test: Documentation of TB skin test or negative QuantiFERON TB test required within 3 months of start date.
9. Health Statement: Physician statement required within 365 days of start date.
10. MGB Criminal Background Check: Required within 45 days prior to start date.
11. MGB Massachusetts CORI: Background check required within 30 days prior to start date.
12. MGB Resume: Must include past 7 years of work experience.
13. MGB Skills Checklist: Required for submission.
14. MGB Confirmation of Assignment/POI Form: Required upon acceptance of assignment.
15. Education Verification: Verification of highest level of education achieved.
16. MGB SSN Trace: Required for background check.
17. MGB National Sex Offender Check: Required within 30 days of start.
18. MGB Exclusion Checks: Required for compliance with OIG, GSA, and MassHealth & NH Medicaid exclusion lists.
