Proof of 3 immunizations and positive antibody titer, if negative titer, booster or Declination accepted.

Must be within 30 days of start. Negative TB skin test, QuantiFERON Gold or T-Spot. Must state negative results. Updated annually. If positive, must show proof of positive history, chest x-ray within last 5 years and annual TB Questionnaire thereafter.

Statement signed and dated within 1 year of start stating traveler is able to work without restrictions. Updated annually.
