  • Required to be sent with file. NC DL must list address greater than 50 miles from facility.
  • Driver’s license must match the state of an RN’s compact licensure. Alternate proof of residency will not be accepted.
  • Required if using a compact license that is not NC. Facilities in this association are strict on this requirement.
