Required Credentials and Documentation

Personal Health History

• MMR Vaccination: Documented proof of two MMR vaccines or positive IGG titer. No declinations accepted.
• Fit Test: Required for all travelers; preferred to be completed prior to start.
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Required during flu season (October 1, 2024 - March 31, 2025). Exemptions accepted with mask requirement.
• PPD/Chest X-Ray: Positive PPD with negative chest X-ray and annual TB questionnaire.
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
• Varicella: Documented proof of two Varicella vaccines or serological evidence of immunity.
• Tdap: Must not be expired; no declinations accepted.
• 12 Panel Drug Screen: Must be completed within 30 days prior to the start of the first assignment.

Professional Credentials

• Driver's License/State ID: Must match address on file and be valid.
• Resume/Profile: Required for submission.
• Professional References: At least two professional references required.
• Criminal Background Check: Must be completed within 30 days of first assignment and updated annually.
• Nursys Verification: Required within 30 days prior to start.
• License/Certificate Verification: Current license verification required within 30 days prior to start.

Additional Requirements

• Ascension Skills Checklist: To be completed within one year of start; updated annually for specialty.
• Ascension Core Competencies: Completed within a year of start and updated yearly.
• Ascension EKG Exam: Must pass with a score of at least 80% prior to clearance.
• Ascension Nursing Contract Worker Onboarding: Required prior to start.
• MOAB Moderate Risk Certification: Required prior to start.
