Adaptive equipment/Community re-entry/pre-discharge planning required

Assessment/Discharge/Evaluation/Plan of care/reassessments required

EPIC experience preferred

Experience in Acute care setting on med-surg, Telemetry and ICU

Skills: Ability to complete, document and synthesize the results of the OT evaluation

OT with strong communication/collaboration with team and other disciplines

Responsibilities: Triaging patients and organizing schedules for the day

Discharge planning meetings if needed

Complete evaluations and make discharge recommendations set on BJC criteria

Timely documentation

Completion of education and competencies (SABA, CPR)

Less frequently ED for discharge recommendations and OP surgery
