Required Credentials and Certifications

1. Driver's License/State ID

   • Required for onboarding.

2. BLS (Basic Life Support)

   • Certification from American Heart Association or Red Cross required.

3. ACLS (Advanced Cardiovascular Life Support)

   • Certification from American Heart Association or Red Cross required.

4. Nursys License Verification

   • Primary source verification of professional license required.

5. COVID-19 Vaccine

   • Proof of 2 vaccines and booster required or declination completed.

6. MMR (Measles, Mumps, Rubella)

   • Proof of 2 vaccines at least 28 days apart or positive titer required.

7. Hepatitis B Vaccine

   • Required; declinations not accepted.

8. Influenza Vaccine

   • Required; may decline but must wear a surgical mask during flu season.

9. TB Test

   • Quantiferon or T-Spot test within the past year with a negative result OR two documented negative PPD skin tests within the past year required.

10. Varicella Vaccine

    • Proof of 2 vaccines at least 28 days apart or positive titer required.

11. Tdap Vaccine

    • Required; declinations not accepted.

12. Physical/Health Statement

    • Must be completed within 60 days of start; must state clear to work.

13. Prime Core Competencies Exam

    • Must score 80% or higher.

14. Prime Skills Checklist

    • Completion required.

15. Prime Criminal Background Check

    • National criminal background check within one year of start date required.

16. Prime Form I-9 (Employment Eligibility Verification)

    • Required for onboarding.

17. Prime Job Description

    • Must be signed by the employee.
