Required Credentials and Qualifications

Certifications

• BLS (Basic Life Support): Required for clinical positions.
• ARRT (American Registry of Radiologic Technologists): Required.

Licenses

• License/Certificate Verification: Primary source verification of professional license and/or certification required within 30 days of start and updated upon expiration. Temporary licenses are not accepted.

Personal Health History

• COVID-19 Vaccine: Proof of vaccine series or completion and approval of client medical or religious exemption form (preferred, not required).
• MMR (Measles, Mumps, Rubella): Positive titers only from within the last 15 years.
• Fit Test: Completed within 1 year of start, updated annually.
• Hepatitis B: Proof of full series or at least two vaccines from the series, or signed declination.
• Varicella: Positive titers only from within the last 15 years.
• Tdap (Tetanus, Diphtheria, Pertussis): Proof of vaccination within the past 10 years.
• 2-Step PPD or IGRA (Quantiferon/Tspot): Negative reads within the last 6 months.
• Influenza Vaccine: Proof of vaccination from October to March or exemptions must be submitted for approval.
• Physical/Health Statement: Completed within one year of initial start date, stating the traveler is able to work without restrictions.

Professional Requirements

• UHS I-9 Attestation: Completed I-9 or attestation that completed I-9 is on file.
• UHS Education Verification: Highest level of education verified; attestations not allowed.
• UHS Employment Verification: Verification of employment within the last 5 years.
• UHS Criminal Background Check Results: Ran within 6 months of start.
• UHS OIG Search: Verified to be ran separately from background check.
• UHS SAM/EPLS/GSA/OFAC: Supporting documentation that the individual was screened and cleared.
• UHS National Database Criminal Background Check: For Ohio residents to meet fingerprint requirements.

Additional Requirements

• Resume: To include education and work history, with 2 professional references required within the last 2 years.
• UHS Badge Photo: Shoulders up, plain background, headshot.
• UHS Module Completion Certificate: Safety and compliance training modules to be completed prior to clearance.
• UHS Skills Checklist: Clinical skills checklist required to be completed within 1 year prior to start.
• UHS Competency Exam: Must score at least 80%, completed within 1 year prior to start.
