Job Description

The Registered Nurse will coordinate and direct home nursing care according to physician's orders, agency policies, and procedures. Responsibilities include:

• Application of the nursing process in home health settings.
• Direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conducting home visits and attending periodic office meetings.

Patient Care

• Average number of patients per day: 5-8
• Types of cases include Start of Care, Wound Care, and Follow-up Visits.
• Points system for productivity: 30 points per week, with each visit and activity holding different point values.

Travel Requirements

• Must have reliable transportation and a current driver's license.
• Willingness to travel up to 75 miles from the home clinic.
• Mileage reimbursed at the IRS standard rate.

Qualifications

• Experience Required: Minimum of 1 year of Adult Home Health or Hospice experience.
• Certifications Required: CPR and BLS.
• On-call Requirements: Varies by location; to be discussed during the interview.
• Weekend Requirements: Varies by location; to be discussed during the interview.
• First-timers Accepted: No, must have recent specialty experience.

Additional Information

• Dress Code: Scrubs
• Charting System: Proficiency in HomeCare HomeBase (HCHB) required; Oasis proficiency for RNs, PTs, and OTs.
• Local Candidates Accepted: Yes, at the same rate.

Compliance and Documentation

Candidates must complete various compliance and health documents prior to starting, including:

• Background checks
• Health documents (e.g., TB test, vaccinations)
• Various facility-specific documents (e.g., confidentiality agreements, compliance training)

Work Schedule

• Standard hours: Monday to Friday, 8 AM to 5 PM.
• No weekend rotation or on-call duties expected.
