Job Description

• Conduct health assessments focused on wound, ostomy, and continence care.
• Perform focused physical assessments and develop care plans based on diagnoses and individual client needs.
• Recommend medications and treatments as appropriate.
• Develop, revise, and evaluate standards of care; assist in selecting supplies, technology, and equipment.
• Assess and document patient responses to treatments for wounds, ostomies, and continence issues.
• Collaborate with the healthcare team and make referrals to other disciplines as needed.
• Educate and counsel patients and families on prevention of complications related to ostomies, wounds, or continence problems.
• Develop and evaluate staff education programs, including new employee orientation and continuing education.
• Conduct comprehensive assessments of patients at risk for pressure injuries and initiate preventative measures.
• Communicate challenges, including ethical issues and conflict resolution.
• Collaborate with the Assistant Director of Nursing for Wound Care to recommend best practices.
• Participate in quality improvement activities within the unit and hospital-wide.
• Provide culturally competent education to patients, families, and staff.
• Maintain confidentiality of patient data and information.
• Engage in professional development activities to maintain competency.
• Identify and assist clients in utilizing community resources.
• Manage assets related to rented equipment, such as specialty beds and negative pressure devices.

Required Experience

• Minimum of 2 years of medical/surgical experience.
• Experience in wound and colostomy care.
