Other Certifications/Specialty certifications requirements: CCM (Certified Case Manager), ACM (Accredited Case Manager), C-SWCM (Certified Social Work Case Manager), CBHCM (Certified Behavioral Health Case Manager); CCM preferred but not required

Case Management - Discharge planning

Will cover all Inpatient units throughout the hospital

Medical discharge could be some social discharges

Experience or 'Must have' skills: Must be RN Case manager with at least 2 years Acute Care Management experience with previous experience in large academic Medical Center hospital

UMMC is the only level 1 trauma hospital in the state so will receive a variety of complex patients and those with no other hospital options

Previous experience in large academic Medical Center hospital

Weekend Requirements: None

Holiday Requirements: Rotated as needed

Department Specific Orientation: 2 weeks (will include general facility orientation)

Will work alongside CM team so will never be working alone

Do you require InterQual criteria or Milliman experience: No, but need to have general understanding of discharge readiness

What areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.): Will cover all Inpatient units throughout the hospital

Social Work team consists of 15 full-time and 17 part-time Social workers

Scrub Color/Dress Code: Any color scrubs or business professional attire

50 mile radius rule

Fingerprint requirement can be done remotely (takes approx 10-14 DAYS) or in person in Jackson, MS 1 week prior to assignment

EMR: EPIC; Will use naviHealth to send referrals and plan to switch to EPIC Care Management over the next year
