Job Description

Position Purpose

To coordinate and implement an individualized plan of care for each patient using the nursing process, practicing within the scope, standards, policies, values, and mission of the organization.

Essential Functions and Basic Duties

• Plans, implements, and evaluates patient care plans based on patient assessment to optimize outcomes.
• Monitors, records, and communicates patient condition as appropriate.
• Collaborates across disciplines to coordinate patient care, including transfer, discharge, referral, and support needs.
• Identifies barriers to effective communication based on psychosocial, literacy, financial, and cultural considerations.
• Evaluates learning needs of patients and/or families and provides appropriate education.
• Effectively delegates patient care to ancillary personnel and may precept student nurses and new hires.
• Employs critical thinking skills and works with minimal supervision in a fast-paced environment.
• Partners with healthcare consumers to implement a patient-centered plan of care.
• Demonstrates caring attitudes and behaviors to develop therapeutic relationships.
• Uses evidence-based interventions and strategies to achieve optimal outcomes.
• Integrates critical thinking and technology solutions.
• Demonstrates respect, equity, and empathy in interactions with culturally diverse and vulnerable patients.

Qualifications

Minimum Requirements

• Must be at least 16 years of age (21 for driving positions).
• Must be legally eligible to work in the United States.
• Must pass a background check and drug screen (if applicable).
• Must complete employee health meeting.

Education/Licensure/Certification

• High School Diploma or equivalent preferred.
• Unencumbered RN License in Colorado or Compact State required.
• Current Certification in BLS required; ACLS and PALS preferred.
• TNCC preferred.

Experience

• Med/Surg experience preferred.
