True Level IV NICU: ELBW (extremely low birth weight) - post-term, birth traumas, critical airways, genetics, surgical needs, NAS, dialysis, cooling, exchange transfusion, trachs, pressors

Certs REQ: BLS, NRP

Additional Notes: Required to be sent with file. NC DL must list address greater than 50 miles from facility. Driver's license must match the state of an RN's compact licensure. Alternate proof of residency will not be accepted.
