Required Credentials

1. BLS Certification

   • Accepted from AHA, Red Cross, CHSF, or Military.
   • Must be current and verified.

2. Certified Surgical Technologist (CST)

   • Certification is mandatory; no exemptions allowed.

3. License/Certificate Verification

   • Primary Source Verification of professional license and/or certification required within 30 days of start date.

4. COVID-19 Vaccine

   • Must have a completed primary vaccination series approved by the FDA or WHO, or an up-to-date COVID vaccine.

5. MMR Vaccination

   • Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine, or proof of immunity via blood test.

6. Influenza Vaccine

   • Required during flu season (Sept 15th - April 30th).

7. TB Test

   • Documentation of TB skin test screening or negative QuantiFERON TB Test within 3 months of start date.

8. Health Statement

   • Physician statement signed by MD/DO/NP/PA required within 365 days of start date.

9. MGB Criminal Background Check

   • Must be completed within 45 days prior to start date.

10. MGB Massachusetts CORI

    • Criminal history check for the state of MA, conducted within 30 days prior to start date.

11. MGB Resume

    • Include past 7 years of work experience with city & state.

12. MGB References

    • Two recent references required, with at least one from a supervisor or above.

13. MGB Skills Checklist

    • Required for submission.

14. MGB Confirmation of Assignment/POI Form

    • Must be updated if there is a change in start date.

15. Education Verification

    • Verification of highest level of education achieved.

16. MGB SSN Trace

    • Must list addresses/aliases for the past 7 years.

17. MGB National Sex Offender Check

    • Conducted within 30 days of start and updated annually.

18. MGB Exclusion Checks

    • Search OIG, GSA, and MassHealth & NH Medicaid exclusion lists.

19. Fit Test

    • N95 fit test required within 365 days of start date.

Additional Information

• Preferred: Documentation of Hepatitis B vaccination or proof of immunity.
• Optional: Tdap vaccination within the last 10 years.
