Required Certifications

• BLS (Basic Life Support) - AHA or ARC Only
• PALS (Pediatric Advanced Life Support) - AHA or ARC Only
• ACLS (Advanced Cardiovascular Life Support) - AHA or ARC Only
• CHS NIH Certification - Annual Requirement

Health and Safety Requirements

• COVID-19 Vaccine - Proof of vaccine and 1 booster or exemptions must be submitted for approval.
• MMR (Measles, Mumps, Rubella) - Proof of 2 immunizations, positive IGG titer, or declination accepted.
• Hepatitis B - Proof of 3 immunizations and positive antibody titer, or declination accepted.
• Influenza Vaccine - Required from October - March (unless otherwise indicated by CDPH).
• TB Test - Must be within 30 days of start; negative results required.
• Varicella - Proof of 2 immunizations, positive titer, or declination accepted.
• Tdap - Proof of vaccination within the past 10 years or declination accepted.

Professional Requirements

• Driver's License/State ID - Required.
• Criminal Background Check - Must be within 30 days of start; updated every 3 years.
• Professional References - 2 required, with at least 1 supervisory reference.
• Education Verification - Diploma, degree, or transcripts required.
• License/Certificate Verification - Primary source verification of professional license and/or certification required.

Additional Information

• HLC Confirmation - Testing to be completed by the Friday before start; stipend available for timely completion.
• CHS Skills Checklist - Skill-specific requirements to be completed within 1 year of start.
• Fit Test - To be completed onsite on the start date.
• Health Statement - Signed statement required, stating the traveler can work without restrictions.
