Job Requirements

• Experience: 2 years of recent experience
• License: WI/Compact License
• Certifications:

  • BLS (Basic Life Support)
  • NIHSS (National Institutes of Health Stroke Scale)
  • ACLS (Advanced Cardiac Life Support)
  • PALS (Pediatric Advanced Life Support)
  • NRP (Neonatal Resuscitation Procedure)
  • NIH Stroke Certification (required for all Med Surg, ICU, and inpatient RN positions)
  • Medication Test/Pharmacology Competency Exam
  • Color Blind Testing (required for candidates working in dialysis/kidney center)

Competencies

• Competency Exam (as applicable to Profession/Specialty)
• Photo (for Badge) - must be a passport-style photo from shoulders up
• Compliance Checklist (to be completed by agency representative)
• Confidentiality Statement (to be signed prior to start)

Health Documents

• Physical Exam (required within 1 year of start)
• Negative TB or Chest X-Ray (must be within 60 days of start date)
• Immunization Records:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug Testing (negative 9 Panel Drug Screen within 1 week of start)
• Flu Seasonal Vaccination/Documentation (required during flu season)

Background and References

• 7 Year Background Check (required within 1 year of start)
• 2 Professional References (verified within 2 years of start)

Additional Information

• Color Blind Exam required for candidates working in dialysis/kidney center.
• Flu vaccine or medical/religious exemption required during flu season (October to March).
• Skills Checklist (required annually per Aya standard).
• WI Caregiver Screening (completed online).
