Job requirements

American Heart Association certification is required for all direct patient care personnel.
American Heart Association certification is required for RNs in CCU, ED and PACU, MICU, SICU.
For any position requiring a license or certification, online verification required within past 30 days.
All engaged contractors must have signed code of conduct form on file.
Standard I9 completed by the agency.
All engaged contractors must have page 5 signed form on file for Confidentiality of Patient & Proprietary Information & Information System Security.
All engaged contractors must have signed Integrity and Audit Services Form on file.
Specialty exam must be completed within a year.
Physical or Health Statement dated within the last year required.
Resume, Skills Checklist within a year, 2 Professional References needed at submission.
Conflict Of Interest Disclosure Form required for all engaged contractors.
Federal, Nationwide, County Criminal check within last 7 years, including SSN Verification within past 30 days.
Education Verification required at onboarding for the highest level of academic education.
EPLS and GSA/OIG exclusions check required within past 30 days.
NSO (National Sex Offender Registry) check required within past 30 days.
