Additional Details

Proof of vaccine and 1 booster or exemptions must be submitted for approval.
Proof of 2 immunizations, positive IGG titer, if negative titer, must have booster or Declination accepted (MMR, Varicella).
Proof of 3 immunizations and positive antibody titer, if negative titer, booster or Declination accepted (Hepatitis B).
Must be within 30 days of start. Negative TB skin test, QuantiFERON Gold or T-Spot. Must state negative results. Updated annually.
Statement signed and dated within 1 year of start stating traveler is able to work without restrictions. Updated annually.
Proof of vaccination within past 10 years or Declination accepted (Tdap).
Required on start date for clinical staff (Covid Test).
HLC Testing to be completed by Friday before Start. Candidates Username and Password will be provided by CMC under the candidates comments. If completed and confirmation provided Friday prior to start, a $300 stipend will be paid.
AHA or ARC Only (BLS, ACLS).
Clinical Only: Skill specific required to be complete within 1 year of start (CHS Skills Checklist).
Proof of vaccine from October - March (unless otherwise indicated by CDPH) or declination (Influenza Vaccine).
