Job Description
- Seeking a Pediatric Nurse with current PALS/ACLS certifications.
Required Certifications and Credentials
- BLS Certification (AHA or ARC only)
- PALS Certification (AHA Only)
- Nursys License (Nursing only, within 30 days of start)
- License/Certificate Verification (Primary Source Verification of professional license and/or certification)
- FMOLHS Skills Checklist (Unit specific to the contractor’s scope of practice within one year prior to start date; updated annually)
- FMOLHS Competency Exam (Copies of exams needed, within year of start with passing score of 80% or higher)
- FMOLHS Core Competencies (Must be current within a year prior to start with passing score of 80% or higher)
- FMOLHS CPI (As required per specialty/Unit & Facility)
Personal Health History Requirements
- MMR Immunization (Proof of 2 immunizations, positive titer or Declination accepted)
- Fit Test (Must be one of the 3 mask fit types, completed within 1 year prior to start)
- Hepatitis B Immunization (Proof of 3 immunizations, positive titer or Declination accepted)
- Influenza Vaccine (Flu Season dates to be communicated each year)
- TB Test (Proof of negative Blood test or Skin test)
- Health Statement (Statement signed and dated within 1 year of start)
- Varicella Immunization (Proof of 2 immunizations, positive titer or Declination accepted)
- Tdap Immunization (Proof of vaccination within past 10 years or Declination accepted)
- Annual TB Questionnaire (Required within 6 months of start)

- FMOLHS Resume (Resume or Profile)
- FMOLHS 2 Professional References (2 references required; 1 Charge or Higher; 1 must be supervisor reference)
- FMOLHS Criminal Background Check Results (Must include various checks and verifications)
- FMOLHS Drug Screen (Negative 10 Panel within 30 days prior to start)
- FMOLHS Badge Photo (Forward facing, well lit, non-blurry Photo with a neutral-colored background)
- FMOLHS Contract Annual Education (See Document Library)
- FMOLHS Eye Exam & Color Vision (Must be Ishihara Color Test within 1 year prior to start)
