Nursing only Michigan hospitals need their licensure verified through LARA not NURSYs but you will still upload here

The Medical Imaging Contract Worker Form will not need to be uploaded prior to start, as it is done onsite and uploaded by the facility
Welcome – Medical Imaging Contract Worker Onboarding. The attestation at the end of the pre-hire 6 hours needs to be uploaded by the Agency once completed, with the travelers’ signature and date
Vaccines or Positive IGG Titer
Exemptions are NOT accepted for persons working in the following areas: Potential contact with pediatrics/infants, mon-immune pregnant women (L&D areas), non-immune immunosuppressed patients (oncology, transplant, etc.)
Flu season starts Sept 4th, 2023 for all new travelers. Needs to be updated every year. Declinations and exemptions not accepted
At the completion of assignment, vendor partner sends survey link to contract worker to be completed within 72 hours. Survery details can be found in the document library
