Welcome – Medical Imaging Contract Worker Onboarding. The attestation at the end of the pre-hire 6 hours needs to be uploaded by the Agency once completed, with the travelers’ signature and date.

Required for all disciplines, Annual

Documentation of vaccination series completion OR documentation of declination to vaccine

1st within 1 year, 2nd within 30 days of start. And/or Negative IGRA within 90 days of offered start date lab printout prior to start.

The candidate or associate should provide documented proof of two Varicella vaccines, serological evidence of immunity, or verification of a history of varicella or herpes zoster (shingles) by a healthcare provider.

Valid 10 years from the administered date.

Qualifying reference(s) completed prior to first assignment. At least two professional references

Include Social Security Trace, Employment Verification, OIG Search and National Sex Offender.

Clinical only; Required annually in specialty.

Within 30 days prior to the start of the first assignment with the client, updated annually thereafter.

Education needs to be validated at the source level in accordance with applicable governing laws.

Exemptions are NOT accepted for persons working in the following areas: Potential contact with pediatrics/infants, mono-immune pregnant women (L&D areas), non-immune immunosuppressed patients (oncology, transplant, etc.).

Flu season starts Sept 4th, 2023 for all new travelers. Needs to be updated every year.

Statement of good health or completed physical exam prior to first assignment, updated annually thereafter

The Medical Imaging Contract Worker Form will not need to be uploaded prior to start, as it is done onsite and uploaded by the facility.
