• Experience Required: 2 years
• Certifications Required:

  • BLS (Basic Life Support)
  • ACLS (Advanced Cardiac Life Support)
  • PALS (Pediatric Advanced Life Support)
  • NRP (Neonatal Resuscitation Procedure)
  • EFM (Electronic Fetal Monitoring)
  • HIPAA Safety Manual Acknowledgement
  • State License Verification
  • NURSYS Verification (for RNs)

Health Documents

• Physical Exam: Required within 12 months of start
• Vaccinations:

  • Tdap vaccine
  • Negative TB or Chest X-Ray within 12 months
  • Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella (Titer or vaccines required)
  • 9 panel Drug Screen within the last 12 months
  • Flu Seasonal Vaccination/Documentation for 2024-25

Background and History Requirements

• Background Check: 7-year criminal check within a year of start
• EPLS/GSA/SAM: Required within 30 days of start
• HHS/OIG: Required within 30 days of start
• References: Proof of 2 clinical references required
• Employment Application: Complete application/resume required

• Photo for Badge: Professional photograph required prior to arrival
• Personal Appearance Policy: Scrubs of any professional design and color; facility provides scrubs for surgical staff
• Code of Conduct Acknowledgement: Must be signed prior to start
