The Acute Rehab / TCU RN focuses on managing complex care needs, promoting recovery, and facilitating the transition of patients from acute care to home or lower levels of care, ensuring optimal outcomes.

Key Responsibilities:

<li>Assess, plan, and provide nursing care to patients in acute rehabilitation and transitional care units recovering from illnesses, surgeries, or injuries.</li>
<li>Collaborate with multidisciplinary teams, including therapists, social workers, case managers, and physicians, to develop and implement individualized care plans.</li>
<li>Monitor and manage patients’ vital signs, pain levels, mobility, and progress toward rehabilitation goals.</li>
