Required Documentation

• Valid Driver's License: Required for all candidates.

Certifications

• ACLS (Advanced Cardiac Life Support): Required for most units; not required for 3 South, 3 West, 4 West, 4 South, and 5 West.
• BLS (Basic Life Support): Required for all candidates.
• PALS (Pediatric Advanced Life Support): Required for most units; not required for ER Clinical Decisions Unit.
• NRP (Neonatal Resuscitation Procedure): May be required for L&D, NICU, and Mother/Baby.
• CHEMO (Chemotherapy): Required for some units; highly preferred for 5 South.
• TNCC (Trauma Nursing Core Course): Required for ER and Surgical & Trauma ICU; not required for ER at CAMC Memorial Hospital.
• CPI (Crisis Prevention Institute): Required for all ER units, Surgical & Trauma ICU, and Behavioral Health.
• Medication Test/Pharmacology Competency Exam: Required for all nursing candidates.

Health Documents

• Physical Exam: Must be completed within 1 year of start and updated annually.
• Fit Mask: Annual fit test required; cannot be completed onsite.
• Vaccinations:

  • Tdap: Required; does not need to be renewed.
  • Flu Vaccination: Mandatory; proof required.
  • Negative TB Test or Chest X-Ray: Required within 1 year of start.
  • Hepatitis B Titer: Required; must provide full series or signed declination.
  • Rubeola, Mumps, Rubella, Varicella: 2 vaccines or positive titers required.

Background Checks

• Criminal Background Check: Required within 30 days of start.
• OIG and SAM Checks: Required within 30 days of start.
• National Sex Offender Registry Check: Required within 30 days of start.

References

• 1 Reference Required: Must be from a Manager, Supervisor, or Charge Nurse from a facility worked within the last year.
